Eligibility

Criteria.
Meeting these eligibility requirements ensures that individuals can access support services tailored to meet their unique needs.
Catchment Area.

Proof of Brain Injury.
Proof of brain injury will be required to access services at BIARD. Staff can assist with locating these documents should you not have them on hand. Acceptable proof of brain injury includes at least 1 of the following:

Medical Documentation

Neurologist or Physician’s Report

Hospital Discharge Summary (Proof of hospitalization and treatment for a brain injury)​

MRI, CT scan, or other imaging results showing evidence of brain injury.

Rehabilitation Records

Documentation from therapy, physical therapy, or cognitive therapy.

Neuropsychological Assessment

Results from a neuropsychological evaluation detailing cognitive, emotional, or behavioral impairments due to the brain injury.

Legal and Insurance Documentation

Workers’ Compensation or Insurance Claims
-
Claims filed for compensation due to a brain injury.

Disability Assessment Reports
-
Reports from a government agency or private insurer confirming the individual’s disability status.

Medical-Legal Reports

Assessments prepared for legal proceedings related to the injury.

*Please note: A doctor's note does not qualify as proof of brain injury; BIARD requires medical documentation with a clear diagnosis.
BIARD does not require a referral from a doctor or medical professional, however referrals are welcome.
After eligibility has been confirmed, an intake will be completed to gain a better understanding of the support you are looking for.
